Diabetes Technology Packages

Structured packages for continuous glucose monitoring and insulin pump therapy, combining the consultations above into a clear pathway.
Tap a package to see the options, then tap an option for full details.
Medicare rebates apply to eligible appointments where eligibility criteria are met; estimated out-of-pocket figures assume those criteria are met.

Continuous Glucose Monitoring (CGM) Start Package

The package includes a clinical review, supply of an appropriate CGM device, CGM set-up and personalised education, followed by a review of your CGM report.

Insulin Pump Start & Upgrade Pathway

Alana is experienced in the initiation and ongoing management of automated insulin pumps approved for use in Australia, and is a Certified insulin pump trainer for MiniMed (Medtronic), mylife Diabetes Care (Ypsomed), AMSL and Insulet. This pathway is for people starting insulin pump therapy for the first time or changing to a different pump.

If you are already experienced with pump therapy and known to Alana, you may only need a pump start and follow-up review rather than the full pathway. Additional appointments may be recommended depending on your pump experience, confidence, clinical needs and the type of pump you are commencing.

Medicare rebates
Eligible consultations attract a rebate.
Many consultations with a Nurse Practitioner attract a Medicare rebate where eligibility criteria are met. The rebate is processed after payment.
Payment
Settled on the day of consultation.
Payment is required at the time of your appointment by EFTPOS or card. Where applicable, the Medicare rebate is typically refunded to your nominated account within 1–2 business days.
Referrals
A referral is not required to make an appointment.
A referral from your GP, specialist or other treating clinician is helpful, as it provides important clinical background and supports a more complete assessment, management plan and communication with your care team.
Cancellation
Please give as much notice as possible.
Appointment times are reserved especially for you. A cancellation fee may apply for appointments cancelled with less than 24 hours' notice. Full cancellation policy will be confirmed at booking.

Please note: package fees and Medicare rebates are subject to eligibility and are reviewed annually, with updates on 1 July each year. Estimated out-of-pocket figures assume Medicare eligibility criteria are met — the exact amount will be confirmed at booking.
