What a consultation looks like.

For your first appointment

  • A referral from your GP or specialist, where relevant
  • A summary of your medications, including insulin types and doses
  • Any recent blood tests (HbA1c, lipids, kidney function)
  • Glucose data from the past two weeks (CGM, meter or logbook)
  • A list of questions or things you would like to discuss

For a review appointment

  • Updated glucose data and any pump or CGM reports
  • Any new medications or changes since we last met
  • Notes on how the agreed plan has been going
  • Any new questions that have come up
